In consideration of being allowed to participate in any way in Menace Hockey Club, LLC activities, the undersigned acknowledges, appreciates, and agrees that:

1. There are certain inherent risks and dangers associated with the sport of ice hockey, including but not limited to, the risk of serious bodily injury or death.

2. The undersigned hereby assumes full responsibility for any risks of injury, damage, or loss that may be sustained by the undersigned or any person for whom the undersigned is responsible while participating in Menace Hockey Club, LLC activities.

3. The undersigned agrees to waive, release, discharge, and covenant not to sue Menace Hockey Club, LLC, its officers, directors, coaches, volunteers, employees, agents, and any other persons or entities acting on its behalf (collectively, the "Releasees"), from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the undersigned or any person for whom the undersigned is responsible, whether caused by the negligence of the Releasees or otherwise, while participating in Menace Hockey Club, LLC activities.

4. The undersigned agrees to indemnify, defend, and hold harmless the Releasees from any and all liabilities, claims, demands, actions, or causes of action whatsoever, including reasonable attorney's fees, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the undersigned or any person for whom the undersigned is responsible, whether caused by the negligence of the Releasees or otherwise, while participating in Menace Hockey Club, LLC activities.

5. The undersigned acknowledges that he or she has carefully read this waiver and release and fully understands its contents. The undersigned voluntarily agrees to its terms and conditions, and understands that this waiver and release is a legally binding agreement that will affect his or her legal rights and remedies.

6. This waiver and release shall be governed by the laws of the state in which Menace Hockey Club, LLC is located.
